Hardwood refinishing involves sanding the upper layer of the wood flooring with the proper machinery...not the stuff you rent but the good quality machines we use.  With another machine we buff out the flooring before and in-between coats.  Sometimes we apply a new stain , and always add multiple clear coats over the flooring. The amount of foot traffic and the kind of movement your flooring sees daily will determine the length of time between each time you need to refinish, normally about once every 5-10 years depending on the use. The more activity and amount of activity the flooring receives. the quicker it wears and fades.
